What Does a Dental Hygienist Actually Do? We Asked Ours

We sat down with Sarah Thomson, our qualified dental hygienist and dental therapist at Greyholme Dental Suite, and asked her the questions patients most want answered. From what a hygienist actually does, to how often you should book an appointment – and what to do if you haven’t been in a while and feel a bit embarrassed about it – Sarah covers it all.

What Does a Dental Hygienist Do?

It is a question Sarah hears often – and the answer might surprise you. “The hygienist role is mainly preventative,” she explains. “And here at Greyholme, we pride ourselves on dealing well with nervous patients. We can acclimatise patients – just get them comfortable if they’re anxious before they see the dentist.”

The key difference between seeing a hygienist and seeing a dentist comes down to focus. While a dentist will carry out your dental health review, take X-rays, and handle restorative work such as fillings, your hygienist is focused on the health of your gums, the prevention of disease, and keeping your mouth in the best possible condition between those appointments. Sarah is also a qualified dental therapist, meaning her skill set goes further than most.

Why Are Hygiene Appointments So Important?

Sarah describes hygiene appointments as a motivating tool as much as a clinical one. “For a lot of people, it acts as a reminder that it’s their job to look after their teeth,” she says. “A lot of patients will never have been shown how to clean their teeth properly. So it’s important because it motivates – and it educates.”

But there’s a clinical reason, too. At Greyholme, every clinician carries out a soft tissue examination at every appointment – regardless of whether the patient has seen the dentist recently or not. “What we’re doing that for is to pick up head and neck cancer,” Sarah explains. “So it’s really important that we’re seeing the patient as a whole. Sometimes we can pick up other systemic problems as well, just from talking about saliva, medication, and general well-being.”

What Are the Early Signs of Gum Disease?

Gum disease is more common than many people realise – and the early signs are easy to miss. Sarah outlines the key warning signals:

  • Bleeding gums – usually the first thing patients notice
  • Red or inflamed gums
  • Halitosis (bad breath) – sometimes noticed by a partner or family member before you notice it yourself

One important caveat: if you smoke, bleeding gums may be masked. “That is why we always ask patients about their smoking and drinking habits,” Sarah says. “Not to nag at them – we do it in a professional way – but because smoking can mask a problem that’s already there.” If you’re concerned about gum disease near Cheltenham or Tewkesbury, a hygienist appointment is the right first step.

How Often Should You See a Hygienist?

For most patients in good gum health, Sarah recommends every six months. “We generally see stable, healthy patients here six-monthly – for a preventative treatment and for the soft tissue examination,” she says.

If you’ve lapsed or you’re a new patient, you may need a few appointments initially to get your gum health stable. And for some patients, three-monthly visits are appropriate. “Those are patients who are immunosuppressed, or diabetic – conditions that put them at higher risk from periodontal disease,” Sarah explains. “Or patients who’ve had periodontal disease previously and need ongoing monitoring.”

Is Brushing Twice a Day Really Enough?

For most people, it can be – but only if technique and diet are both on point. “If done correctly, and if the diet is perfect – which I don’t think anyone’s could possibly be – then twice a day might be enough,” Sarah says. “But from a fluoride point of view, for someone at higher risk from decay, they’re missing out on that extra hit of fluoride, as well as plaque removal through the day.”

What’s the Biggest Misconception About Dental Hygiene?

Sarah doesn’t hesitate: it’s the idea that dental hygiene is the hygienist’s responsibility, not the patient’s own. “A lot of people think we’re going to tell them off. But the biggest misconception is that it’s just to do with the mouth – whereas now we know there are links to lots of other issues.”

The research increasingly connects periodontal (gum) disease to wider health conditions, including heart disease and dementia. “The same bacteria implicated in periodontal disease are implicated in these other medical scenarios,” she explains. “Sometimes people assume it’s just the mouth we’re looking at. But quite often it’s that we’re picking up on other things as well.”

Your gum health, in short, is your whole-body health.

How Can You Improve Your Oral Health Between Appointments?

Sarah’s team takes a very hands-on approach to this. “We’re always happy to help patients tweak their routine, or for them to send us little clips if they’re worried that something isn’t working properly,” she says. “Because sometimes when we show somebody something at the beginning of a treatment plan, there will be improvement during the course of the treatment – and we may need to adjust what they’re using.”

Between visits, Sarah recommends:

  • Using disclosing tablets to identify areas you’re missing
  • Keeping an eye on bleeding patterns – any increase is worth flagging
  • Being mindful of your diet, including frequency of sugary foods and drinks
  • Involving children and the whole family in the routine – good habits start early

What If You Feel Embarrassed About Your Teeth or Gums?

This comes up more than you might think. Sarah’s response is warm and matter-of-fact: “I usually say that I’ve seen worse. There is no shame – we’re not here to embarrass people, we’re here to help them. It’s none of our business why they haven’t been. We’re just delighted that they’re back.”

How Does Professional Cleaning Help Prevent More Serious Treatment?

It comes down to access. A toothbrush can clean to around 2-3mm below the gum line – but for patients with more advanced gum disease, pockets can be deeper than that. “Professional cleaning gets to the areas that patients simply can’t reach themselves,” Sarah explains. “Not just between teeth, but underneath the gum.”

The BPE (basic periodontal examination) we carry out at each visit checks specifically for these deeper areas. Catching and treating them early is far less involved – and far less costly – than letting things progress.
